SUN SPOTS

No one wants them, but you can certainly help to prevent them and there are also several successful treatments available to get rid of them. But let’s start with explaining how you get them.

Cumulative sun exposure throughout the earlier part of your life triggers skin cells called melanocytes to start to produce more pigment, as they are being activated by the sun’s U.V. rays. The more regularly your skin is being exposed to the sun, the more likely you will have them as you get older (which by the way is why they are also known as “age” spots).

Patient 1

Patient 2

Patient 3
Sun spots, also called liver spots or age spots, are commonly found on the face, backs of hands, arms & legs, backs, shoulders and chest as these areas are more likely to be exposed to the sun, probably even on a daily basis. Avoiding sun exposure can be challenging, especially living in sunny California, but you can certainly limit your sun exposure and be sure to always wear your sun screen — yes, I said it again.

Sun spots are more common in skin that burns easily, and if you’ve had enough sun exposure to acquire sun spots, you have a higher risk of developing skin cancer. The spots themselves are harmless, however should you notice any of the spots changing or becoming larger please seek medical attention to rule out any pre-cancer or cancerous lesions. Also if you develop a few spots and just aren’t sure whether or not they are actually sun spots please see your doctor to make that determination.

Unfortunately since most of us are pretty much past the age to completely prevent sun spots, there are several options for treating them:

Active-FX/Deep-FX Laser Treatments
A new type of fractional laser ideal for treating aged, scarred & sun damaged skin. The laser’s energy is focused on deeper tissues of the skin to correct and rejuvenate the skin as well as improve the overall texture of the skin. Deep-FX laser resurfacing can also help to reduce the appearance of lines and wrinkles in the skin & improve skin tone and texture.

IPL (Intense Pulsed Light)

A great skin rejuvenation treatment using broadband light which delivers energy to both superficial and deeper layers of the skin, this both treats and reduces the dark pigment and redness in the skin (called melanin). IPL helps to smooth the skin, and fade sun spots, freckles and melasma.

Cosmelan Peel
A lightening treatment mask suitable for all skin types containing active ingredients to lighten existing discoloration and can inhibit discoloration deeper in the skin from surfacing.

Chemical Peels
We offer several chemical peels in our office and medi-spa, such as the TCA Peel, Skin Medica (Vitalize Peel), Glycolic Peel & an organic Green Peel. Not sure which is suitable for your skin? Meet with someone on our medical staff to help you decide. Chemical peels treat and remove sun damage, melasma and skin discoloration from acne as well as treat fine lines and wrinkles. Chemical peels help to lighten the skin gradually, so usually a few are recommended for maximum results.

*MAINTAIN RESULTS BY BEING SUN-SAFE: ALWAYS USE SUNSCREEN, LIMIT YOUR SUN EXPOSURE AND BE SURE TO WEAR A HAT WHEN YOU ARE PLANNING TO BE OUT IN DIRECT SUNLIGHT.

MELASMA

Melasma (sometimes called pregnancy mask) is a darkening of the facial skin which can be caused by estrogen supplements, birth control pills, pregnancy and sun exposure. Like sun damage or age spots, melasma is also the result of overactive melanocyte cells over-producing pigment in the skin. Melasma is usually seen on the cheeks, mid forehead, jaw line and around the mouth. Melasma is more commonly seen in darker skins, or skin that has a tendency to tan such as African-American, Asian & Hispanic skin-types.

Melasma can be treated successfully. There are several treatments available but because treating melasma effectively can vary from patient to patient, sometimes it’s a matter of finding the treatment that’s the best fit: Treatments recommended might be a Cosmelan peel, a chemical peel, IPL or laser treatments, or topicals like a bleaching cream containing hydroquinone or Tri-Luma. Sometimes a combination of treatments are best, like chemical peels done in conjunction with micro-dermabrasion and bleaching creams over a period of a few months prove effective

Remember, once you have achieved desired results from melasma treatments it is very important to continue with preventative maintenance; that means definitely wearing a sunblock every day, avoiding excessive sun exposure, wearing protective clothing and hats when necessary and perhaps a bleaching cream at night.

A Patient's Perspective

Mary Strong
Mary Strong of NFL Sports:

“The sun took a major toll on my skin after playing pro-beach volleyball for many years. And then I found Dr. Shamban and all the wonderful staff at the Laser Institute. They worked with me to turn back the clock. Now, when I look in the mirror I honestly can’t believe how amazing my skin looks and feels! I hardly have to wear make-up anymore and I get tons of compliments on my skin. I feel like I’m working with a team to achieve the best results. It’s made all the difference in the world.”

“I had a lot of sun damage from playing pro-beach volleyball. Dr. Shamban and the staff at the Laser Institute recommended Active FX to help reduce sunspots and the fine lines around my eyes. After only 4 days down time I couldn’t believe my eyes! My skin looked like it did when I was 20! No lines, no freckles, just beautiful radiant skin. I don’t even need to wear make-up anymore. I am very impressed with the results!”

MYTH BUSTERS: SUN SPOTS

MYTH 1: Is it true that my sun damaged skin will repair itself & the sun spots will go away if I just stay out of the sun from now on?

Absolutely not. Remember most of us have been probably exposing our skin to too much sun for most of our lives. Unfortunately by the time we are in our 30’s and 40’s we are starting to show it. This is the time to really become careful with our skin, taking every possible precaution to avoid exposing our skin to the sun. The only way to repair and treat sun damaged skin is to seek treatment like lasers, photofacials or peels, and after treatments do your best to avoid sun exposure, period. It’s pointless to have these procedures and then go right back to your old routines and just have that sun damaged skin return and worsen. Be proactive year-round!

MYTH 2: Are age spots and freckles just part of the aging process?

No. Sun spots and freckles are a result of over-exposure to the sun. Your chances for ending up with sun spots increase with increased sun exposure, meaning the more time you spend in the sun younger in life makes you that much more likely to develop sun spots later in life. You are not getting the sun spots simply because you are getting older.

MYTH 3: If I have a peel or some kind of treatment for melasma or brown patches will it ever come back?

Yes it can come back, even with preventative maintenance after treatments. However if you are diligent about caring for your skin after your treatments but find your melasma does come back you can keep it from worsening with proper care. After treating your melasma I usually recommend patients continue using some type of bleaching cream, Retin-A, or continuing peels or photofacials periodically to keep the skin clear. These things in addition to daily sunscreen and avoiding direct sun exposure will help your melasma stay away. Sometimes pregnant women will find that post-pregnancy their melasma does improve or go away, and occasionally other melasma sufferers do notice an improvement without treatment, but this is rare.

DID YOU KNOW…

Did you know that there are treatments for melasma safe to do during pregnancy? Most importantly take care to make sure the melasma doesn’t get any worse by wearing a good sunblock every day and wear a sun-protective hat if you’re out in the sun.

SilkPeel and Microdermabrasion treatments are safe during pregnancy and SkinCeuticals makes a great topical treatment for excessive pigmentation called Phyto + Botanical Gel. With botanicals like cucumber extract, kojic acid, and thyme extract, Phyto+ is a natural alternative to products containing hydroquinone, and it’s active ingredients work synergistically to help lighten the skin. You can also use a gentle exfoliating scrub like Nia-24 once a week to treat yourself to soft, smooth skin while promoting cell turnover for fresher, brighter skin.

Ask Dr. Ava
Q: Help! I’m already stressed-out thinking about the holiday parties I’ll have to go to this year because my skin’s not looking so great. What can I do to kind of perk-up my skin? I need to look great by Christmas.

Great question. Everyone wants to look great for the holidays and there are actually several things you can do. I’d recommend you start with a SkinMedica Peel for a really good exfoliation, and then a week to two weeks later a microdermabrasion treatment, that way you’re skin is all prepped and ready to go, and I find that my patients get better results this way. Then you should have at least two IPL treatments about 3 weeks apart, and lastly treat yourself to a luxurious facial a week or so before your parties. You’ll look and feel fabulous & you’re certain to get on Santa’s good list!
